The Biden administration distributed 1,200 doses of smallpox vaccine to people who were at high risk of the virus, part of the national public health response to eradicate the disease before it caused a serious outbreak.
Concerned that the virus is spreading faster than previously thought, U.S. health officials say the global monkeypox epidemic is the largest ever. The World Health Organization said on Wednesday that there are currently more than 550 cases in 30 countries. In the United States, at least 20 confirmed or suspected cases have been reported in 11 states, including California, Colorado, Florida, Georgia, Illinois, Massachusetts, New York, Pennsylvania, Virginia, Utah and Washington, according to the Centers for Disease Control and Prevention.
“An outbreak of monkeypox of this magnitude and scope has never been seen before,” Dr Raj Punjabi, who heads the White House pandemic preparedness service, told reporters last week.
However, CDC officials have tried to reassure the public that the arrival of monkeypox in the United States is significantly different from Covid-19, which blinded the country two years ago. Scientists knew little about Covid when it first appeared and the United States did not have vaccines or antiviral treatments to fight the virus in 2020.
Monkeypox, on the other hand, has been known to scientists since 1958, when the virus was first identified during epidemics among monkeys kept for research purposes, and its transmission in humans has been studied since the 1970s. . Global health authorities also have extensive experience in the successful fight against smallpox, which the World Health Organization declared eradicated in 1980 following successful global vaccination efforts. Accumulation of vaccine stocks

The United States has two vaccines, but the preferred option is in smaller quantities. Jynneos is a two-dose vaccine approved by the FDA in 2019 to prevent monkeypox in people aged 18 and over. The CDC usually recommends Jynneos over the other option, ACAM2000, which is an older generation smallpox vaccine that can have serious side effects.
Last week, CDC spokesman Dr. Jennifer McQueston said the United States had 1,000 doses of Jynneos. However, the Danish biotechnology company that makes the injections, Bavarian Nordic, said the United States actually has more than 1 million frozen doses of Jynneos stored in the United States and Denmark on an order made in April 2020. The United States has ordered nearly 30 million doses of Jynneos since 2010, but 28 million have expired, the spokesman said. Bavarian Nordic plans to increase production this summer and has the capacity to produce 30 million photos a year, the spokesman said.

“We want to ensure that people at high risk have quick access to vaccines and, if they get sick, can receive appropriate treatment,” Punjabi told reporters on Friday. The ACAM2000 demonstrates high levels of monkey pattern protection in animal models and is expected to provide 85% protection against diseases from the virus, similar to earlier versions of the vaccines against smallpox, according to Mike Sliveka, an immunologist from the University of Health and Science in Oregon who studied monkeypox. Less is known about Jynneos because the vaccine is newer, but it produces reasonable levels of antibodies in humans and must protect against serious illness, Slifka said.
Side Effects
The CDC usually recommends Jynneos over ACAM2000 because it is considered safer. ACAM2000 could have serious side effects, and widespread distribution of the vaccine would require serious discussion, McQueston told reporters last week. ACAM2000 uses a mild viral strain from the same family as monkeypox and smallpox, which can still be replicated, which means that there is a risk that the live virus in the vaccine will spread to the human body or to other people.

CDC Warning
The CDC said that pregnant or lactating women, people with weak immune systems, those with skin conditions such as eczema or atopic dermatitis, and people with heart disease should not receive ACAM2000. Mortality
Smallpox can have a mortality rate of up to 30%, according to the WHO. The West African strain of monkeypox, which appears to be driving the current epidemic, is likely to have a mortality rate of around 1%, although data are scarce, as the virus has previously spread mainly to remote parts of Africa. Most people recover within two to four weeks without specific medical treatment, according to the CDC.
